CHD approved forms are included on the reverse side of
your printed Health forms included in the Summer Brochure.
In interest of download speeds, these forms are available
for download separately. YOU MUST download both forms
bring them with you or your child on the day of registration
(first day of camp).
Your child may not attend camp without these forms.
